-Provides nutrition services using established standards for transfer of information and unique patient identifiers.
-Implements Medical Directive if in place (i.e. insulin initiation and adjustment).
-If appropriate, delegates the implementation of the Nutrition Care Plan to the Dietetic Assistant and/or Food Service employee.
-Monitors and adjusts care plans based on Department/Program Nutrition Standards of Care and/or caseload volume.
-Communicates care plan in written/electronic health record using organization and College documentation standards.
-Applies adult education techniques to develop and facilitate group and education programs.
-Provides individual nutrition care plans, teaching and follow-up employing adult education techniques.
-Provides patient and/or caregiver with appropriate teaching materials that are geared to reading level, readiness to change and are culturally appropriate.
-Provides education related to patient safety using the teach-back method.

Job description: POSITION SUMMARY:
The Registered Practical Nurse (RPN) is a regulated health care professional who adheres to and models the Standards and Guidelines of the College of Nurses of Ontario; the SJHH mission, vision and values, policies and procedures; and applicable specialty standards. The RPN provides patient/client-centred and evidence informed care to promote health and assess the need for the provision of care and the treatment of health conditions by supportive, preventative, therapeutic, palliative and rehabilitation means in order to attain or maintain optimal patient/client function. The RPN collaborates with the inter-professional team in the delivery of a full range of care and treatment to patient/clients.

QUALIFICATIONS:

  • Current registration with the College of Nurse of Ontario required.
  • Education in one of the below categories is required:

a) Practical Nursing Diploma from an accredited Community College program with evidence of IV medication administration as part of the basic educational curriculum.
b) Practical Nursing Diploma from an accredited Community College program and IV medication administration certificate from an accredited Community College program if not included in basic diploma curriculum.
c) Practical Nursing Certificate from an accredited Community College program, a medication administration certificate and an IV medication certificate from an accredited Community College program

  • Minimum of 2-years current surgical/medical clinical experience required
  • Current Basic Cardiac Life Support (BCLS) certification required.
  • Demonstrated ability to function to the full scope of nursing practice per the College of Nurse of Ontario.
  • Demonstrated ability to communicate effectively (written and verbal) with patients/clients, families and other members of the health care team.
  • Demonstrated ability to set priorities and organize work in response to client/patient needs.
  • Demonstrated commitment to ongoing learning, professional development, and continuous quality improvement
  • Basic skill level with computers, MS Word, Internet searches, data entry and patient information systems
  • Participation and successful completion of 9 week in house Peri-operative Certificate Program.
  • RPNAO or other professional association membership preferred

Specific Requirements:
Applicants must successfully complete internship offered by St. Joseph’s Healthcare in collaboration with Mohawk College.

HOURS OF WORK:
Full time hours required internship period and clinical orientation for approximately 20 weeks. Hours of work will include rotating shifts. Shifts vary from 7.5 to 10 hours and include weekends, evenings and nights, holidays and on-call.
