Title: Licensed Practical Nurse (LPN) – Part Time

Location: Landerbrook Transitional Care, Mayfield Heights, Ohio, United States

Work Schedule: Part-time, 16-24 hours per week, 2nd and 3rd shifts

Job Description:
Landerbrook Transitional Care is seeking a dedicated and dependable Licensed Practical Nurse (LPN) to join our team on a part-time basis. In this role, the LPN will be responsible for providing quality care to our residents under the supervision of a registered nurse.

Primary Job Responsibilities:
– Administer medications and treatments as per physician’s orders
– Monitor and record vital signs and changes in residents’ condition
– Assist with resident assessments and care planning
– Provide wound care and dressings as necessary
– Collaborate with the interdisciplinary team to ensure resident care needs are met
– Document all care provided in a timely and accurate manner
– Communicate effectively with residents, families, and care team members
– Adhere to all facility policies and procedures as well as state and federal regulations
– Maintain a clean and organized nursing station and resident care areas

Qualifications:
– Valid Ohio LPN license
– Current CPR certification
– Strong clinical skills and knowledge of nursing practices
– Exceptional communication and teamwork abilities
– Compassionate and caring attitude towards residents
– Ability to prioritize tasks and work efficiently in a fast-paced environment

Experience:
– Minimum of 1 year of experience in a long-term care or skilled nursing facility preferred
