Position: Case Manager

Location: Toronto, ON

Salary: $55,000 – $65,000 a year

Description:
Our growing organization is currently seeking a Case Manager to join our team in Toronto. The ideal candidate will have a passion for helping others and a strong background in social work or a related field. The Case Manager will be responsible for providing support and assistance to clients in need, helping them navigate various social services and resources available to them. This position requires excellent communication and organizational skills, as well as the ability to work effectively with a diverse range of clients.

Responsibilities:
– Conduct assessments and develop individualized care plans for clients
– Coordinate services and resources for clients, including housing, healthcare, and financial assistance
– Monitor client progress and provide ongoing support and advocacy
– Collaborate with community organizations and agencies to ensure clients receive the best possible care
– Maintain accurate and up-to-date client records
– Participate in team meetings and trainings as required

Qualifications:
– Bachelor’s degree in social work or a related field
– Previous experience working as a Case Manager or in a similar role
– Strong communication and interpersonal skills
– Knowledge of community resources and social services
– Ability to work independently and as part of a team
– Valid driver’s license and access to a reliable vehicle
