Clinical Case Manager

We are seeking a passionate and experienced Clinical Case Manager to join our team. This position will involve providing case management services to individuals with mental health and substance abuse disorders. The ideal candidate will have strong assessment and treatment planning skills, as well as the ability to coordinate and monitor care for clients. Responsibilities include conducting assessments, developing individualized treatment plans, collaborating with community resources, and providing crisis intervention as needed. The Clinical Case Manager will also be responsible for documenting client progress and participating in regular team meetings to discuss client care. Qualified candidates should have a minimum of a Bachelor’s degree in social work or a related field, as well as relevant experience in case management. A valid driver’s license and the ability to travel to various client locations is also required.
