Job Description

Clinical Pharmacist

Unichem Ponsonby has a fantastic opportunity for a motivated and dynamic Clinical Pharmacist to join their team. This role involves providing a range of clinical pharmacy services to patients, supporting a multidisciplinary healthcare team, and promoting best practice in medication management.

Key Responsibilities:
– Conducting medication reconciliation and patient medication reviews
– Providing pharmaceutical care services such as medication therapy management
– Collaborating with healthcare professionals to optimize patient outcomes
– Participating in clinical ward rounds and patient case conferences
– Monitoring medication usage and responding to drug information queries

Qualifications and Skills:
– Bachelor of Pharmacy degree
– Current Pharmacy Council registration and practicing certificate
– Minimum of 2 years of clinical pharmacy experience
– Strong communication and interpersonal skills
– Ability to work effectively in a multidisciplinary team environment
