Description:

We are currently seeking a Case Manager to join our team in Boston, MA. The Case Manager will be responsible for assessing clients’ needs, developing care plans, and providing ongoing support to ensure clients receive appropriate services. The ideal candidate will have experience in case management, strong communication and organizational skills, and the ability to work effectively with diverse populations.

Responsibilities:
– Conduct initial assessments to determine clients’ needs
– Develop individualized care plans in collaboration with clients and other service providers
– Coordinate service delivery and referrals for clients to meet their identified needs
– Monitor clients’ progress and make adjustments to care plans as needed
– Provide ongoing support, advocacy, and crisis intervention as needed
– Maintain accurate and up-to-date client records
– Collaborate with team members and external partners to ensure holistic care for clients

Qualifications:
– Bachelor’s degree in social work, psychology, or related field required
– 2+ years of experience in case management or related field preferred
– Strong communication and interpersonal skills
– Ability to work effectively with individuals from diverse backgrounds
– Knowledge of community resources and social service agencies
– Ability to work independently and as part of a team
– Bilingual language skills a plus
