Clinical Pharmacist

We are seeking a dedicated and experienced Clinical Pharmacist to join our healthcare team. The ideal candidate will be responsible for providing comprehensive pharmaceutical care to our patients, including medication therapy management, drug information services, and oversight of medication safety.

Responsibilities:
– Review medication orders and ensure appropriateness and accuracy
– Collaborate with healthcare providers to develop and implement individualized medication therapy plans
– Monitor patient responses to medication therapy and make recommendations for adjustments as necessary
– Provide medication counseling and education to patients
– Participate in multidisciplinary healthcare teams to improve patient outcomes
– Ensure compliance with all pharmacy regulations and standards of practice

Qualifications:
– Doctor of Pharmacy (PharmD) degree
– Current pharmacist license in the state of practice
– Board certification in a clinical pharmacy specialty preferred
– Strong clinical knowledge and critical thinking skills
– Excellent communication and interpersonal skills
– Ability to work independently and as part of a team
