Position: Occupational Therapist – Case Manager

Location: Kitchener, ON

The Occupational Therapist – Case Manager is responsible for providing client-centered occupational therapy services within a community-based program. In this role, the individual will complete initial and ongoing assessments, develop treatment plans, and provide therapeutic interventions to clients to promote their independence and meaningful engagement in daily activities.

Key Responsibilities:
– Conduct comprehensive occupational therapy assessments to identify client needs and goals
– Develop individualized treatment plans in collaboration with clients and other team members
– Provide evidence-based therapeutic interventions to address physical, cognitive, emotional, and functional needs
– Monitor client progress and adjust treatment plans as needed
– Provide education and support to clients and their caregivers on strategies to promote independence and well-being
– Collaborate with other healthcare professionals and community agencies to ensure coordinated care for clients
– Maintain accurate and timely documentation of client assessments, treatment plans, and progress notes

Qualifications:
– Bachelor’s or Master’s degree in Occupational Therapy from an accredited program
– Registered with the College of Occupational Therapists of Ontario
– Minimum 2 years of experience working as an occupational therapist in a community-based setting
– Experience working with individuals with a variety of physical and cognitive disabilities
– Knowledge of assistive devices, home modifications, and community resources
– Strong communication, problem-solving, and organizational skills
– Ability to work independently and as part of a multidisciplinary team
– Valid driver’s license and access to a vehicle
