Job Description:

We are seeking a detail-oriented and compassionate Care Coordinator to join our team at ReachFirst. As a Care Coordinator, you will be responsible for managing and coordinating the care of our clients, ensuring that their needs are met in a timely and effective manner.

Key Responsibilities:

– Conducting initial assessments of clients’ needs and developing individualized care plans
– Coordinating and scheduling services such as home care, therapy, and other supportive services
– Communicating with clients, their families, and healthcare professionals to ensure the delivery of high-quality care
– Monitoring and evaluating the effectiveness of care plans and making necessary adjustments
– Providing support and guidance to clients and their families throughout the care process
– Maintaining accurate and up-to-date documentation of client care

Qualifications:
