The company is looking for a Manager/Estimator with experience in project management within the construction industry. The candidate should have a strong understanding of construction processes and will be responsible for overseeing project management.
Registered Nurse (RN) – Integrated Care Manager

Our healthcare facility is seeking a Registered Nurse (RN) to join our team as an Integrated Care Manager. The ideal candidate will have a strong background in nursing and case management, with a passion for providing high-quality patient care.

Responsibilities:
– Coordinate and oversee care for patients with complex medical conditions
– Develop care plans and treatment goals in collaboration with patients and their healthcare team
– Provide education and support to patients and their families
– Monitor and evaluate patient progress, adjusting care plans as needed
– Serve as a liaison between patients, healthcare providers, and community resources
– Participate in interprofessional team meetings to discuss patient care and outcomes

Qualifications:
– Registered Nurse (RN) license in good standing
– Bachelor’s or Master’s degree in Nursing preferred
– Previous experience in case management or care coordination
