Position: Social Worker – Case Manager

Location: San Antonio, TX

Job Description:

We are seeking a passionate and dedicated Social Worker to join our team as a Case Manager. In this role, you will be responsible for providing comprehensive case management services to individuals and families in need. You will work closely with clients to assess their needs, develop treatment plans, and connect them to community resources.

Responsibilities:

– Conduct assessments to determine clients’ needs and develop appropriate treatment plans
– Provide individual and group counseling to clients
