Job Description:

We are seeking a dedicated Patient Care Coordinator to enhance patient care and ensure the effective delivery of services in our healthcare setting. This role involves:

• Assisting with outreach and marketing programs aimed at local hospitals and EMS systems to promote our services and build strong relationships within the community.
• Collaborating with healthcare professionals to improve patient experience and streamline care processes.
• Providing support and guidance to patients through their care journey, ensuring their needs are met effectively and compassionately.
• Engaging in community outreach efforts to raise awareness about available services and resources.

Key Responsibilities:

• Develop and implement marketing strategies to promote services to hospitals and EMS systems.
• Maintain communication with healthcare partners to foster collaborative relationships.

• Collect and analyze patient feedback to improve service delivery and patient satisfaction.

Qualifications:
