Job Description: Nurse Navigator

We are seeking a dedicated and proactive Nurse Navigator to join our team. The Nurse Navigator plays a crucial role in enhancing patient care by providing guidance and support throughout the healthcare journey.

Key Responsibilities:

• Awareness Building: Actively participates in marketing initiatives and community outreach programs to promote the Nurse Navigator role and its benefits to patients and families.

• Patient Engagement: Engages with patients and their families to explain the Nurse Navigator’s role, ensuring they understand the support available to them.

• Collaboration: Works closely with healthcare providers, community organizations, and stakeholders to create partnerships that enhance patient resources and accessibility.

• Education: Develops educational materials and conducts presentations to inform patients and the community about available services and the importance of navigation through healthcare services.

• Feedback Gathering: Collects feedback from patients and the community to assess awareness and effectiveness of outreach efforts and to improve services.

Qualifications:

• Bachelor’s degree in Nursing (BSN) required; advanced degree preferred.
• Current nursing license and relevant clinical experience.
• Strong communication and interpersonal skills.
• Experience in marketing or community engagement is a plus.
