We are seeking a passionate and dedicated Nurse Navigator to join our team. The Nurse Navigator plays a crucial role in guiding patients through their healthcare journeys, ensuring they receive comprehensive support and resource access. This position involves active participation in marketing and community outreach efforts to raise awareness of the Nurse Navigator role and its benefits.

Key Responsibilities:

• Engage in targeted marketing initiatives to promote the Nurse Navigator program within the community.
• Collaborate with local organizations and healthcare providers to establish partnerships and outreach opportunities.
• Develop educational materials and presentations to inform patients and their families about available resources and support services.
• Obtain or create patient-centric resources tailored to address specific needs and barriers to care.
• Act as a liaison between patients, families, and healthcare professionals, fostering communication and understanding.
• Conduct assessments to identify individual patient needs and connect them with appropriate services.
• Track and report on outreach efforts and patient outcomes to continuously improve programs and initiatives.

Qualifications:

• Bachelor’s degree in Nursing (BSN) required; Master’s degree preferred.
• Active RN license in the state of employment.
• Strong communication and interpersonal skills.
• Experience in community outreach, patient education, or marketing is a plus.
• Ability to work collaboratively in a multidisciplinary team environment.
