Job description: Tri-County Mennonite Homes is looking for a Registered Dietician to support our Nithview Community in New Hamburg and our Greenwood Court location in Stratford.
The Registered Dietician will play a crucial role in enhancing the quality of life for residents and contributing significantly to their well-being and overall health.
This position will be reporting to both the Director of Culinary Services at Nithview Community and the Director of Culinary Services, Housekeeping and Laundry at Greenwood Court.
This role will work 72.5 Hours monthly. 67% of the time will be spent at Nithview Community, and 33% of the time will be spent at Greenwood Court. This position is based on the Ministry of Long-Term Care guidelines, and the hours could increase in the future.
Responsibilities
- Complete Resident Nutrition Risk Assessments for all new admissions
- Conduct quarterly and annual nutrition assessments to help determine individual dietary needs, preferences, and any specific health conditions
- Assign appropriate diets to optimize and liberalize meals for residents, considering their health goals, and based on their assessments
- Prescribe therapeutic diets as needed, such as low sodium or renal diets based on regulations
- Ensure that residents receive the right dietary modifications to manage specific health conditions
- Actively work to prevent malnutrition among residents, monitor nutritional intake, address deficiencies, and promote adequate nourishment. If malnutrition is identified, develop personalized strategies to improve residents’ nutritional status.
- Embrace the philosophy of person-centered values and consider residents’ preferences, cultural backgrounds, and individual needs when planning meals to ensure that dietary recommendations align with residents’ unique circumstances and preferences.
- Collaborate with physicians, nurse practitioners, and the entire Care Team to enhance residents’ quality of life. Educate residents, staff, volunteers, and families on nutrition-related matters, supporting individualized nutrition care.
- Collaborate with the resident’s attending physician regarding the nutritional plan of care and associated assessment needs, i.e.: lab results, vitamin supplementation, diet changes, etc.
- Review all resident weights on a monthly basis with the Care Team and initiate appropriate interventions and documentation as necessary
- Review and/or update resident diet cards, nourishment lists, diet census sheet and dining seating plans as requested by the Director of Culinary Services
- Participate in relevant clinical teams and resident care conferences, as required
- Other related duties required or assigned
Qualifications:
- Have an Undergraduate Degree in Food and Nutrition, or equivalent, from an approved University.
- Successfully completed an accredited Dietetic Internship
- Current registration with the College of Dietitians and member of Dietitians of Canada
- Possess experience in clinical nutrition and geriatrics.
- Excellent verbal and written communication skills
- Proficient with Microsoft Office Suite
- Experience with PointClickCare and MealSuite software is a plus
- Excellent organizational skills
- Strong problem solving and decision making skills
- Ability to work well with a cross-functional team
- Knowledge and understanding of RAI-MDS and Ministry of Long-Term Care standards and guidelines
