- Successful completion of dietetic internship program accredited by Dietitians of Canada required
- Current registration in good standing with the College of Dietitians of Ontario required
Current certification as a Certified Diabetes Educator (CDE) (willingness to obtain certification within a prescribed time frame) required
- Current certification as a Certified Nutrition Support Clinician (CNSC) preferred
- Current membership in Dietitians of Canada preferred
- Motivational Interviewing Training preferred
- Experience in adjusting insulin preferred
- Insulin Pump Experience an asset
- Certified Insulin Pump Training (willingness to obtain certification within a prescribed time frame) required
- Evidence of sound organizational and prioritization skills
- Demonstrates a commitment to customer service when dealing with patients, colleagues, volunteers and other members of the community and public
- Engages in ethical conduct and strives to understand and appreciate the diversity of our patients, staff population and community
- Experience leading quality improvement projects, data reporting and reviewing an asset
Skill, Ability and Experience:
- Demonstrated skill in nutrition support (enteral and parenteral) preferred.
- Demonstrated ability to provide clinical nutrition education to paediatric, adult, and geriatric patient populations required
- Demonstrated skill in nutrition care in diabetes education (Type 1, 2, GDM, Pre-Diabetes), eating disorders, renal, and mental health with paediatric, adult, and geriatric patient populations) required
- Experience with glucometers, insulin adjustments, and insulin pump education required
- Intermediate level proficiency with Microsoft Office required
- Demonstrated experience working with electronic medical record required
- Capacity to work within a multidisciplinary patient care team required
Principal Duties:
- Triage and arrange appropriate referrals in collaboration with other dietitians and DEC clinicians.
- Patient Nutrition Assessments
- Evaluate patient lab results pertinent to nutrition intervention
- Calculate nutrition requirements
- Identify patients’ need for follow up and perform patient follow ups in person, by e-mail or by telephone or virtual
- Identify education needs and assess readiness to change of patients and family members
- Develop appropriate teaching plans, provide education, and evaluate learning
- Participate in patient care interdisciplinary team meetings
- Routine communication with administration, food services, allied health, and physicians
- Employ information technology appropriately for patient care (i.e. statistical tracking)
- Document patients’ nutrition care in patients’ records
- Stay current and aware of opportunities to implement new, evidenced-based methods of patient assessment and treatment
- Participates in unit/department and Hospital Quality Improvement initiatives
- Completes continuing education requirements as necessary
- Willingness to attend professional development opportunities (including weekends and evenings)
- Other related duties as assigned
