Skill, Ability and Experience:

  • Demonstrated skill in nutrition support (enteral and parenteral) preferred.
  • Demonstrated ability to provide clinical nutrition education to paediatric, adult, and geriatric patient populations required
  • Demonstrated skill in nutrition care in diabetes education (Type 1, 2, GDM, Pre-Diabetes), eating disorders, renal, and mental health with paediatric, adult, and geriatric patient populations) required
  • Experience with glucometers, insulin adjustments, and insulin pump education required
  • Intermediate level proficiency with Microsoft Office required
  • Demonstrated experience working with electronic medical record required
  • Capacity to work within a multidisciplinary patient care team required

Principal Duties:

  • Triage and arrange appropriate referrals in collaboration with other dietitians and DEC clinicians.
  • Patient Nutrition Assessments
  • Evaluate patient lab results pertinent to nutrition intervention
  • Calculate nutrition requirements
  • Identify patients’ need for follow up and perform patient follow ups in person, by e-mail or by telephone or virtual
  • Identify education needs and assess readiness to change of patients and family members
  • Develop appropriate teaching plans, provide education, and evaluate learning
  • Participate in patient care interdisciplinary team meetings
  • Routine communication with administration, food services, allied health, and physicians
  • Employ information technology appropriately for patient care (i.e. statistical tracking)
  • Document patients’ nutrition care in patients’ records
  • Stay current and aware of opportunities to implement new, evidenced-based methods of patient assessment and treatment
  • Participates in unit/department and Hospital Quality Improvement initiatives
  • Completes continuing education requirements as necessary
  • Willingness to attend professional development opportunities (including weekends and evenings)
  • Other related duties as assigned
