Job Description:Salary Range: $47.29 – 56.30Position Summary:The Social Worker collaborates with their clients to address challenges through a process of assessment, diagnosis, treatment and evaluation of individual, interpersonal and societal problems. Facilitating continuity of care, the Social Worker, applies social work knowledge, skills, interventions and strategies, to assist individuals and families in an acute care and inpatient alternative level of care (ALC) setting to achieve optimum psychosocial and social functioning.You will be assisting the patient and the family in developing and enhancing coping skills related to the social and emotional needs and problems that may accompany or predate illness/injury and hospitalization. You will refer patients to wide variety of internal and community partners. You have excellent Clinical skills with demonstrated current professional practices and standards of discipline. You demonstrate excellent interpersonal skills when liaising with the interdisciplinary health care team, external community partners as well as patients and their families. You also have excellent critical thinking, organizational, conflict resolution skills as well as excellent listening and communication skills, both verbal and written.The social workers at Cambridge Memorial Hospital have expertise in the areas of general medicine, surgery, critical care, geriatrics, rehabilitation, oncology, palliation, obstetrics and paediatrics.As the Professional Practice Leader (PPL) you will provide leadership for members of their profession to meet their regulatory college standards of practice and code of ethics for the patient population served. Reporting to the Manager, Professional Practice and the Clinical Manager.The PPL collaborates with clinical managers and will be responsible for the leadership of professional practice projects, programs and initiatives. The PPL will focus on continuously improving practice in alignment with organizational priorities and policies, including support for regulatory requirements for competent and safe practice, patient and family centered care, documentation, confidentiality, and code of conduct.The PPL provides educational services to social work students, mentorship and coaching for staff and clinical internship placements from accredited universities.The physical demands of the job may include but are not limited to prolonged sitting, standing, walking, bending, pushing, pulling and lifting. This position contributes to ensuring there is a safe environment for patients, staff and visitors.Skills and Qualifications:
- Masters preparation and registration with the Ontario College of Social Workers and Social Services Workers Social Work (MSW, RSW) required.
- A minimum 3-5 years work experience in a hospital setting is preferred.
- Working knowledge of the health system and relevant legislation (e.g.. Mental Health Act of Ontario, Child & Family Services Act, Health Care Consent Act) is required.
- Strong skills in community resource linkage, and collaborative and innovative care planning within a multidisciplinary team is required. Familiarity with related local community resources is an asset.
- Interprets and foresees the implications of impending legislation, professional standards and guidelines from government and professional bodies. In collaboration with team members, develops, reviews or updates discipline specific policies and procedures.
- Chairs their profession specific meeting and facilitates the ongoing support of professional practice, education and research reflective of CMH practice standards, accreditation standards and specific college requirements.
- Works with the Manager, Professional Practice when liaison with the regulatory college is required.
- Ensures essential elements of professional practice standards are consistent throughout CMH.
- Consults with the Manager, Professional Practice and Human Resources to identify discipline specific credentialing requirements.
- Participates in recruitment and onboarding processes, including orientation and contributes to feedback and/or performance reviews.
- Acts as community liaison for discipline specific College and professional associations including the coordination of student mentorship programs.
- Excellent organizational skills with the ability to prioritize workload and meet workload requirements.
- Excellent communication skills (both verbal and written).
- Demonstrated ability to attend work on a regular basis is required.
