This includes but is not limited to:

  • Review medication orders for safety and appropriateness, and verify orders in an electronic computerized health information system
  • Provide drug information and clinical pharmacy support (e.g. pharmacokinetic monitoring, etc.)
  • Responsible for the oversight of drug distribution services in collaboration with other pharmacy staff (including IV medications, TPN, chemotherapy, narcotics etc.)
  • Assisting in the investigation and reporting of medication related incidents
  • Ensure provision of pharmacy services in compliance with accepted standards and relevant legislation (accreditation, Can. Soc. of Hosp Pharmacists, Food & Drug Act, Regulated Health Professions Act, Public Hosp. Act, NAPRA, USP 797, etc.)
  • Effectively communicate with nursing, medical staff, pharmacy team members and management
  • Act as immediate supervisor and resource person to pharmacy technicians, students, etc.

EDUCATION & CERTIFICATION(S)

  • Graduate of a degree program in Pharmacy from a recognized University (B. Sc Pharm. or PharmD)
  • Holds a current license from the Ontario College of Pharmacists
  • Member in good standing of the Canadian Society of Hospital Pharmacists an asset

REQUIRED SKILLS & EXPERIENCE(S)

  • Three (3) to five (5) years’ experience as a Pharmacist working in a hospital pharmacy department an asset
  • Management experiences an asset
