Dietitian Nutrition Support Practitioner – Permanent Full Time

Position Summary:
Reporting to the Manager, the successful candidate will be responsible for providing medical nutrition therapy to patients requiring both enteral and parenteral nutrition, as well as those requiring therapeutic diet instruction and/or education. The successful candidate will work as part of a team to develop, implement and evaluate patient care plans, and will provide ongoing patient/family education and support.

Key Responsibilities:
– Assessing patients’ nutritional status and developing individualized care plans
– Providing nutrition support through enteral and parenteral nutrition as required
– Monitoring patients’ progress and modifying care plans as needed
– Educating patients and families on therapeutic diets and nutrition-related topics
