Job Description:
As a Support Worker, you will be responsible for providing person-centered support to individuals with disabilities and complex needs. Your main duties will include assisting with daily living activities, promoting independence, and providing emotional and practical support. You will work closely with individuals, their families, and other professionals to ensure the highest standard of care and support.
Key Responsibilities:
-Providing support and assistance with daily living activities such as personal care, meal preparation, and household tasks
-Promoting independence and enabling individuals to make choices and decisions
-Establishing and maintaining positive relationships with individuals, their families, and support networks
-Implementing individual support plans and documenting progress and outcomes

Requirements:
