Community Psychiatric Nurse

Position Description:
– Working as part of a dynamic multi-disciplinary team, the Community Psychiatric Nurse will provide high-quality care and support to service users with severe mental illness.
– Conducting comprehensive assessments of service users and working collaboratively with them to develop care plans and interventions.
– Delivering evidence-based therapeutic interventions and support to service users, including medication management and health promotion activities.
– Monitoring service user progress, evaluating outcomes, and adjusting care plans as necessary.
– Collaborating with other healthcare professionals, community agencies, and families to ensure holistic care for service users.
– Engaging in crisis intervention and risk assessment as necessary.
– Maintaining accurate and timely documentation of all interactions and interventions.
– Actively participating in team meetings, clinical supervision, and continuous professional development activities.

Qualifications:
– Registered Nurse with current registration and in good standing with the local regulatory body.
– Minimum of 2 years of experience working in mental health services.
– Experience working with service users with severe mental illness, including complex needs, dual diagnosis, and forensic histories.
– Strong assessment and care planning skills.
– Ability to work independently and as part of a team.
– Excellent communication and interpersonal skills.
– Valid driver’s license and access to a vehicle for community visits.
– Commitment to ongoing professional development and reflective practice.
